                                 PASSPORT SERVICES

Full and limited validity passports are issued by the American Citizens Services Unit.   
In Thailand, persons seeking a United States passport must appear in person at the
Consulate to apply. To make an appointment for a passport service, please visit the
appointment page.

                 You may renew your old Passport at any time

If you are older than 16 years, and you are replacing a U.S. Passport that was issued
after you turned 16, you will need:

Download, or complete online, Form DS-82

1. Form DS-82, Application for U.S. Passport

* Please complete the "Can I Use This Form?" section on the cover page of the      
DS-82. If you answer "no" to any of those questions, please see the instructions for
"First-Time Adult Passport," below.
* Please complete all items to the best of your ability. If you cannot remember                
some information, please check your records or ask family or friends for                        
assistance.
* Include a local phone number, so we can contact you in case of a problem with            
your application. Also, be sure to include an e-mail address so that we are                   
able to contact you to let you know when the new passport is ready.
* When you have completed the application, print it and sign it.

2.  Your most recent U.S. passport

3.  Photographs
: Two identical, unretouched, full-face photographs taken within the
last six months.  The photos should be two inches square, in color, on a white
background.  See the passport photo document for dimensions.

4.  Application fee: $110.00.  You may pay in U.S. dollars or the equivalent in Thai
baht.  The Consulate accepts Visa and MasterCard.  

                 FIRST-TIME ADULT PASSPORT

If this is your first U.S. passport since reaching 16 years of age, you will need:

1. Form DS-11, Application for U.S. Passport  Down Load Here

* Please complete all items to the best of your ability. If you cannot remember                
some information, please check your records or ask family or friends for                      
assistance.
* Include a local phone number, so we can contact you in case of a problem with            
your application. Also, be sure to include an e-mail address so that we are able to
contact you to let you know when the new passport is ready.  When you have
completed the application, print it, but do not sign it.

2.  Proof of U.S. citizenship: Please bring one of the following original documents:

- Government-issued birth certificate from the United States.
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Certificate of Citizenship

3.  Proof of identity:  A valid, government-issued photo ID, such as a driver's license,
U.S. or foreign passport, military ID, or government employee ID.

4.  Photographs: Two identical, unretouched, full-face photographs taken within the
last six months.  The photos should be two inches square, in color, on a white
background.  See the passport photo document for dimensions.

5.  Application fee: $135.00.  You may pay in U.S. dollars or the equivalent in Thai
baht.  The Consulate accepts Visa and MasterCard.  

                 LOST OR STOLEN PASSPORT

If you are replacing a lost or stolen U.S. Passport, you will need:

1. Police Report

* Contact the Tourist Police by calling 1155. They will direct you where to report             
the loss or theft.
* If you have difficulty communicating with the police, ask your hotel, guest house,          
or tour company to help.
* Be sure to get both the Thai and English copies of the police report.
* Keep the police report on you at all times. Without your passport, it is your                   
primary form of identification.

* When you have completed the application, print it, but do not sign it.

3.  Form DS-64, Statement Regarding a Lost or Stolen Passport

* Please provide as much detail as possible. You must indicate whether the                    
passport was "lost" or "stolen." When you are done, sign the form.

* When you come to the Consulate to apply for the replacement passport, you               
must swear or affirm under oath that all of the information that you provide about        
the circumstances of the loss or theft is true and complete. We will enter this              
information into the U.S. government's passport database.

4.  Photographs: Two identical, unretouched, full-face photographs taken within the
last six months.  The photos should be two inches square, in color, on a white
background.  See the passport photo document for dimensions.

5.  Proof of identity:  If you have it or can get it, please bring any type of
government-issued photo ID.

6.  Proof of U.S. citizenship: If you are able to, please bring one of the following:
An original, government-issued birth certificate from the United States
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Certificate of Citizenship

7.  Application fee: $135 for adults and $105 for those under 16 years of age.  You
may pay in U.S. dollars or the equivalent in Thai baht.  The Consulate accepts Visa
and MasterCard.

If you are under age 16, please see "Minor Passport," below, for additional application
requirements.  

                                 MINOR PASSPORT

If you are under age 16, you will need:

1. Form DS-11, Application for U.S. Passport  Down Load Form Here  

* Please complete all items to the best of your ability. If you cannot remember                
some information, please check your records or ask family or friends for                 
assistance.
* Include a local phone number, so we can contact you in case of a problem with            
your application. Also, be sure to include an e-mail address so that we are able           
to contact you to let you know when the new passport is ready.
* When you have completed the application, print it, but do not sign it.

2.  Proof of U.S. citizenship and parentage: Please bring one of the following
original documents:
*  Government-issued birth certificate from the United States that lists parents
*  Consular Report of Birth Abroad
* Certificate of Naturalization (Please also bring an original of your                                  
government-issued birth certificate).
* Certificate of Citizenship (Please also bring an original of your                                      
government-issued birth certificate).

3.  Both parents
* Each parent must have a valid, government-issued ID, such as a driver's license         
or passport.  
* If either parent cannot attend, he or she should execute Form DS-3053,                      
DownLoad Form Here Statement of Consent: Issuance of a Passport to a                   
Minor under Age 16 .
This form must be notarized.

* If only one parent appears on the birth certificate, that parent should come.
* If only one parent or legal guardian has sole custody, that person should come.          
Please bring original of the court order or decree granting custody.
* If the order or decree is not in English, please also bring a certified translation.

4.  Photographs: Two identical, unretouched, full-face photographs taken within the
last six months.  The photos should be two inches square, in color, on a white
background.  See the passport photo document for dimensions.

5.  Application fee: $105.00.  You may pay in U.S. dollars or the equivalent in Thai
baht.  The Consulate accepts Visa and MasterCard.

Please allow 14 days for processing after we receive your passport application.
Passports are printed in the United States, and they are then sent to the Consulate.  
As soon as the passport arrives, we will notify you by e-mail with instructions for
pick-up.
